AHC Caregiver Navigator Plan Process
After completing the one-time payment of $299.00, you'll proceed through the following steps:

Initial Contact:

You'll receive a phone call from an AHC Service Provider to schedule a virtual or phone appointment. This session will be recorded for your convenience.
Medication Assessment:

You'll be provided with Medication Assessment Forms, including consent forms authorizing AHC to discuss your medications with you and your Primary Care Physician.
Health History Review:

A licensed medical provider will conduct a thorough review of your health history and current medications, asking pertinent questions to ensure accuracy.
Personalized Medication Plan:

Based on the assessment, a customized medication recommendation plan and resource summary will be developed for you to review with your Primary Care Physician. AHC will also send a letter to your physician with these recommendations.
